Begin with an outcome you can evaluate
A traceability chain starts with a business reason that can be examined. “Modernize finance” is too broad to explain individual delivery choices. Identify the capability change, intended benefit, accountable owner, and measurement approach. Record the assumptions behind the investment so later teams can understand why the work was funded and when the case needs revisiting.
Link decisions rather than every sentence
Connect the outcome to the initiative, material requirements, design decisions, delivery work, and acceptance evidence. Prefer a small set of meaningful relationships that people will maintain over exhaustive links created once and abandoned. Each link should help answer a question: why is this requirement necessary, which decision does this test support, or what benefit does this release enable?
Use the chain when scope changes
When a requirement changes, identify the affected design, test, and expected outcome. The decision-maker can then see whether the change is local or alters the investment logic. A supplier validation change, for example, may require new acceptance evidence and operating ownership even when the implementation looks small. Update the rationale as part of the scope decision rather than leaving it for final documentation.
Distinguish delivery completion from value
Passing a test demonstrates something about the implemented behavior; it does not by itself prove adoption or a business benefit. Assign a benefit owner and a measurement window after release. Record external factors that could influence the result. A target should remain a target until evidence supports a measured outcome, and the explanation should remain available when performance differs from the original case.
Check the weak links
Sample important decisions and ask whether their sources and downstream consequences can be found. Track orphaned requirements, unsupported outcomes, and stale relationships.
